One interesting approach that is used is to playtest the entire game from start to finish regularly (with new people each time), because if you play parts in isolation you never know whether the player would have acted the same way had they had the prior experience of the rest of the game. A puzzle that seems extremely hard on its own might be a piece of cake given preceding 'training'...
